- Two application forms
- Two annual report forms
- One green form
Forms available at Town Clerk's office.
Fee is $10.00. A permit is valid for one year from the date of issuance.
Only non-profit organizations in existence a minimum of two years are eligible to obtain a raffle permit. First-time applicants must submit a statement of organization or bylaws.
