Instructions to Applicants

New Construction Perc Tests
  • Testing dates are to be scheduled by your engineer with our Title 5 Agent - Your engineer will receive an email from our office with instructions.
  • The application must be filed at the Board of Health office with the required fee prior to testing.
  • Application is valid for one (1) year from date of issue. Fees are non-refundable.
  • Percolation and deep hole test results are good for a two (2) year period, after which time application for an extension may be made.
  • The results of all Percolation tests must be filed in the Board of Health office.
Plan Review
  • The applicant must file the application for a Plan Review, 5 sets of plans, the application for a Disposal Works Construction Permit, and the Permit Verification form at the Board of Health office with the required fees prior to review.
  • Application for Disposal Works Construction Permit must be completed by your engineer and signed by the installer.
  • Massachusetts General Law allows a forty-five (45) day review period on septic system design plans. Upon completion of the review, your engineer will be notified by the Board of Health. Please do not contact the Board of Health office.

Disposal Works Construction Permit

  • Application for Disposal Works Construction Permit and the Permit Verification form must be filed at the Board of Health office with required fee at the time of Plan submittal.
  • The installer must be licensed in the Town of Sutton.The installer must work off a plan showing  Board of Health approval stamp.
  • The Disposal Works Construction Permit must be paid for and pulled prior to the beginning of construction. You will also pay for the Inspection fee at this time.
Inspections & Certificate of Compliance
  • The installer shall contact the design engineer and Board of Health office to request an inspection
  • The As-built form must be completed before the Certificate of Compliance can be signed by our engineer.
  • Four (4) sets of As-built plans must accompany the Certificate of Compliance.
  • It is the obligation of the applicant to obtain all of the necessary signatures. This form must be completed before the town agent will sign.
  • The Property Owner will be contacted to pick up their final paperwork.
